Clinical manifestations of chronic renal failure, such as edema, dysuria, frequent vomiting and dysuria, toxin urine, consumptive disease, hiccups, all belong to the scope of Traditional Chinese Medicine. In〝Visceral Qi on Current Exposition of Plain Questions〞, There is a theory, that is, the person with renal disease has a big belly, edema of legs, heavy body and cough with gravity asthma. The gravity and symptoms of this disease have been expounded in 〝Jingyue’s complete works, Dysuria〞. It says that less urine and difficult passing urine is considered as Dysuria. This is the most dangerous emergency. Retention of urine would cause bloated spleen and stomach, swollen muscle, middle energizer emesis, upper energizer dyspnea. Several days of urinary retention will make the patient feel not only intolerable, but also is in great danger. In 〝Supplement to Diagnosis and Treatment〞,the theory of the most dangerous situation is that the patient with frequent vomiting and dysuria, inevitably, discharges urine difficultly, vomits suddenly. Blocked up by turbid pathogenic factors, the patient has difficult ascending and descending of congenital essence of triple energizer. As a result, difficult passing urine makes the patient vomit. All these symptoms mentioned above are similar to the theory of chronic renal failure.
There are four kinds of pathological mechanism, that is, deficiency, turbidity, stasis and pathogen in the process of developing this disease. Among them, deficiency is the main problem, and kidney is the center of the disease. It endangers liver, spleen and lungs. As the development of the disease, Yin will damage Yang or Yang will damage Yin up to kidney, spleen and liver turn to Yin and deficiency or spleen,kidney turn to Yang, then becoming deficiency and failure. At the same time of asthenia healthy Qi and sthenia pathogenic factors, more sthenia pathogenic factors of stasis, turbidity and pathogen are forced to produce. Regardless of deficiency of both Qi and Yin or deficiency Yang of spleen and kidney, any of them can cause kidney to lose balance and Qi activity of triple energizer to lose its function. The food taken in can’t be turned into nutrient, but changed into damp pathogen, then, ascending and descending of Qi will be abnormal, urine is turbid and can’t be discharged, which will cause to be unable to take in the nutrient of food, but loss of nutrient. All these factors contribute to the disease. Because of tedious course of disease to lose function of Qi mechanism, soon, blood veins are blocked. This is the so-called obstructed blood vein by long term of disease. Because renal urine turbidity and spleen function disorder cause damp pathogen retention to obstruct the Qi activity of triple energizer and urine passage, which helps to endanger blood veins, Deficiency can’t overcome pathogenic factors, so it causes disease. Just because of kidney Qi deficiency, it is easy for the patient to be attacked by pathogenic factors. In a word, asthenia healthy Qi is the root and asthenia pathogenic factors is the sign of the disease. The four great factors are the cause and effect of the disease. It is they that cause vicious circle.
